Community Borrowing Card Application/Renewal Form

Use this form to request or renew any UW-Madison Libraries Community Borrowing Card, including an Annual Fee Card, a Courtesy Card, or a Friends of the Library Card.

You will receive an email when your card is ready for pick-up. We estimate it will take 3 business days to process applications and renewals.

Once you get your email notice, you will be able to pick-up the card anytime Memorial Library is open.  If you had a borrowing card in the past, this card will be deactivated upon receiving the notice the new card is ready for you to pick-up.

Note that a Community Borrowing Card does not provide off-site access to electronic resources.

Wisconsin Residents – FEE CARD

Requirements: Must be a current Wisconsin resident. The annual fee is $40 ($20 for those 62 or older). All cards expire June 30 of the current fiscal year. The fee is not prorated. You will receive an email when the card is ready. When you pick up the card at Memorial Library, you will need to present a current Wisconsin State issued ID (driver’s license or state ID) along with payment. Cash is accepted Monday - Friday, 8 am - noon and 1 – 4 pm. Credit cards (MC and Visa only) are accepted anytime the library is open. This card does not provide off-site access to electronic content.

UW Alumni - FEE CARD

Requirements: Must be a current Wisconsin resident. The annual fee is $40 ($20 for those 62 or older). All cards expire June 30 of the current fiscal year. The fee is not prorated. Beloit College Faculty or Staff

Requirements: Must be full- or part-time permanent staff of Beloit College. Excludes limited term employees. You will receive an email when the card is ready. When you pick up the card at Memorial Library, you will need to present a current state or government issued ID (passport, driver's license, state ID), along with verification of eligibility. Verification may be a letter from Beloit Human Resources of new appointment or being listed in the online directory.

Beloit College Students

Requirements: Letter on Beloit College letterhead obtained from Beloit College librarian verifying current enrollment and authorizing the student to obtain borrowing privileges. You will receive an email when the card is ready. When you pick up the card at Memorial Library, you will need to present a current state or government issued ID (passport, driver's license, state ID), along with the letter.

City of Madison Employees (excludes limited term employees)

Requirements: Must be contributing to the Wisconsin Retirement System. You will receive an email when the card is ready. When you pick up the card at Memorial Library, you will need to present a current state or government issued ID (passport, driver's license, state ID), along with one of the following:
1. a letter from the City of Madison Human Resources department regarding a new appointment;
2. a current paystub showing you are contributing to the Wisconsin Retirement System;
3. being listed in the online directory.

Dane County Employees (excludes limited term employees)

Requirements: Must be contributing to the Wisconsin Retirement System. You will receive an email when the card is ready. When you pick up the card at Memorial Library, you will need to present a current state or government issued ID (passport, driver's license, state ID), along with one of the following:
1. a letter from Dane County Human Resources regarding a new appointment;
2. a current paystub showing you are contributing to the Wisconsin Retirement System;
3. being listed in the online directory.

Edgewood College Faculty or Staff

Requirements: Must be full- or part-time permanent staff of Edgewood College. Excludes limited term employees. You will receive an email when the card is ready. When you pick up the card at Memorial Library, you will need to present a current state or government issued ID (passport, driver's license, state ID), along with verification of eligibility. Verification may be a letter from Edgewood Human Resources of new appointment or being listed in the online directory.

Edgewood College Students

Requirements: Letter on Edgewood College letterhead obtained from Edgewood College librarian verifying current enrollment and authorizing the student to obtain borrowing privileges. Friends of the Library

Requirements: Must be a current member of the “Friends of the Library.” Demonstrate membership by presenting a receipt of an online donation through UW Foundation to “Friends of the Library Fund,” or must be listed in the current Friends of the Library Directory. For further information, please refer to the Friends of the Library. You will receive an email when the card is ready. When you pick up the card at Memorial Library, you will need to present a current state or government issued ID (passport, driver's license, state ID), along with verification of eligibility.

Madison College Faculty or Staff

Requirements: Must be full or part-time permanent staff of a Madison College campus located in the city of Madison. Excludes limited term employees. You will receive an email when the card is ready. When you pick up the card at Memorial Library, you will need to present a current state or government issued ID (passport, driver's license, state ID), along with verification of eligibility. Verification may be a letter from Madison College Human Resources of a new appointment or listing in the online directory.

Madison College Students

Requirements: Must present letter on Madison College letterhead obtained from Truax or Downtown Madison College librarian authorizing student to obtain borrowing privileges until June 30 of current fiscal year. When you pick up the card at Memorial Library, you will need to present a current state or government issued ID (passport, driver's license, state ID), along with the letter.

PLATO - Participatory Learning and Teaching Organization current member

Requirements: Must be listed as a current PLATO member on the PLATO website. You will receive an email when the card is ready. When you pick up the card at Memorial Library, you will need to present a current state or government issued ID (passport, driver's license, state ID).

State of Wisconsin Employees (excludes limited term employees)

Requirements: Must be contributing to the Wisconsin Retirement System. You will receive an email when the card is ready. When you pick up the card at Memorial Library, you will need to present a current state or government issued ID (passport, driver's license, state ID), along with one of the following:
1. a letter from State Employees Department’s Human Resources of new appointment;
2. a current paystub showing you are contributing to the Wisconsin Retirement System;
3. being listed in the online directory.

Teachers - Wisconsin Public Schools (grades 4K-12)

Requirements: Must be a current employee of a Wisconsin Public School District. Excludes limited-term employees. You will receive an email when the card is ready. When you pick up the card at Memorial Library, you will need to present a current state or government issued ID (passport, driver's license, state ID), along with one of the following:
1. a letter from the School District’s Human Resources Department of new appointment;
2. a current paystub showing you are contributing to the retirement system;
3. being listed in a Wisconsin Public School’s directory such as Madison Metropolitan School District.
Private school teachers must obtain a Wisconsin Resident Fee Card.

UW Madison Faculty, Staff, or Student’s Spouse or Domestic Partner

Requirements: Must present one of the following items for proof of relationship:
1. a joint mortgage, lease or title;
2. designation of partner as beneficiary for life insurance or retirement contract;
3. durable property or healthcare powers of attorney;
4. joint ownership of motor vehicle, joint checking account or joint credit account;
5. marriage certificate;
6. current health insurance card listing both spouse/domestic partner names on it.
Will also need UW-Madison spouse’s/domestic partner’s name with their Wiscard number to verify current affiliation - enter that information below. You will receive an email when the card is ready. When you pick up the card at Memorial Library, you will need to present a current state or government issued ID (passport, driver's license, state ID), along with verification of eligibility.
