Please fill in the form below to place a duplication request for UW-Madison Archives materials. Please note this form is for University Archives materials only. Please contact the relevant other library unit or the Wisconsin Historical Society for their specific duplication policies.

If you have not yet identified specific materials to request for duplication (for example, you only have a topic or subject in mind), please first contact University Archives staff (uwarchiv@library.wisc.edu) to help you find specific materials.

Please use this form ONLY If you need a copy of materials and you cannot download a copy on your own. Please note in some cases, you can download a copy of the material on your own. If you have done so or already have a copy, you do not need to request duplication in this form. Only request duplication in this form if you need Archives to retrieve a copy for you.
